Connectus Health currently provides physiotherapy services for prospective NDIS participants and NDIS participants who are NDIA, self managed, plan managed. Our physiotherapists are able to provide ongoing therapy in line with participant goals as well as conduct functional assessments to support your NDIS application or review.
What is the National Disability Insurance Scheme (NDIS)
The NDIS is a scheme by the government that aims to provide opportunities to people living with permanent and significant disabilities through funding to help them achieve their goals enabling them greater independence and participation.
Participants, their loved ones and support teams decide where to allocate their funding to support and service providers that will help to build their capacity and independence.
Who is eligible to receive funding on the NDIS?
The NDIS supports those with a significant disability, who are between the ages of 7 to 65 years.

Functional capacity assessment
How is NDIS funding managed?
NDIS funds can be managed in one of three ways:
1. The NDIA provides you with funding so you can access the supports that will best help you pursue your goals. This is called self-managed funding.
2. The NDIA will provide funding in your plan to pay for a Plan Manager who pays your providers for you, helps you keep track of funds and takes care of financial reporting for you. This is called plan-managed funding.
3. The NDIA pays your providers on your behalf. This is called NDIA-managed funding.

I’m on the NDIS how do I access the services I need?
The first step will be to find a Service provider who can complete the functional assessment. If you have a support coordinator, they can assist you with finding a provider.
A list of registered NDIS providers can be found on the NDIS website if your plan is NDIA managed, or if you are self or plan managed you can utilise any provider of your choice. You can then book an initial appointment with your provider of choice.
What does getting started with a service provider look like?
1. Prior to our initial assessment, we will speak with the participant and, if applicable, their support coordinator to establish what you are looking to achieve from the session and any other necessary information to ensure a seamless and goal centred initial session.
2. At your initial appointment, your therapist will conduct an assessment to identify any functional limitations and devise a plan to work with the participant to improve or overcome some of these limitations.
The initial assessment will generally include the following things:
An assessment of a participant’s functional capacity in relation to their daily activities and community participation.
Understanding the goals and what the participant is looking to achieve with our therapy sessions.
We will discuss the potential structure and timing of your therapy and consider any other supports that you could access which may be beneficial in achieving your therapeutic goals within your plan dates. This allows most efficient allocation of funds for optimal outcomes.
What do I get after the initial session?
After the session we will provide you with the following:
Agreed goals that have been identified together between participant and clinician.
A report outlining assessment findings and our recommended plan to achieve your goals.
A guide of the recommended hours of servicing required.
A service agreement for ongoing physiotherapy services that you can review and decide if you would like to continue on your journey with us.
